Patti L. Walker
President and CEO
As president and CEO of Continuum Home Health’s sister organization Continuum of Care, Inc., Patti supervised Continuum Home Health’s formation and startup in 2001. Her responsibilities at Continuum Home Health include ensuring coordination and integration of home care and psychiatric support programs, and she directs the agency’s policies and operational management. Prior to her time at Continuum of Care and Continuum Home Health, Patti worked for a family counseling agency and managed a program for developmentally challenged adults.
Patti’s ongoing work with Continuum of Care provides a direct connection with that organizations’ deep experience in providing crisis and support services to persons with physical and psychiatric challenges. In addition, Patti’s background in family counseling and social work helps keep Continuum Home Health focused on developing each patient’s potential as individuals and as members of the community.
Patti received a bachelor’s degree in social work from Southern Connecticut State College in New Haven, a bachelor’s degree in sociology and social work from Central Connecticut State University in New Britain, and a master’s degree in social work administration from Fordham University at Lincoln Center in New York City.
Kathy L. DellaMura, R.N.
Vice President
Kathy has been with Continuum Home Health since 2001, and has served as the agency’s vice president since 2004. She directs Continuum Home Health’s clinical services, including clinical staffing, compliance with state and federal requirements.
Kathy’s background as a homecare case manager, wellness consultant, nursing supervisor and director of a geriatric health care center helps ensure that Continuum Home Health’s programs are tailored to patients’ specific needs.
Kathy holds degrees in nursing from St. Vincent’s Medical Center School of Nursing in Bridgeport and healthcare administration from St. Joseph’s College in Windham, Maine.
